SAFETYmatters - Incident Reporting

The Department of Risk Management, Insurance and Loss Prevention is responsible for administering risk management and insurance programs for the University of Iowa. This includes insurance and self-insurance programs covering property and liability risks. Incidents involving injuries to unpaid students and volunteers should be reported through the Incident Information Form in Risk Management.

Risk Management encourages members of the University of Iowa community to report all campus incidents to Risk Management that affect the general public, students, or others that may reasonably result in a liability claim against the University. Incidents to be reported include an injury to a student, faculty, staff, and/or campus visitor, slip and fall, accidents, near-misses where harm could have been caused. The incident report form can be found on the Risk Management website. The form may be completed electronically and submitted.

Risk Management is available to consult or collaborate with individuals, student groups, departments or administration on issues related to the management of risks on campus.
